Access to Medical Records
- Patient Access: Patients or their authorized family members (spouse or children with written consent) can request copies of their medical records.
- Deceased Patient Access: The executor of the estate or next of kin can request copies of the deceased patient’s medical records with a letter of authorization and a copy of the death certificate.
- Request Procedure:
- An official request form must be completed.
- The Attending Physician’s consent is required for all requests.
- Record Release:
- Photocopies are stamped with the Hospital seal and signature of the issuing personnel.
- Requests are processed within 48 hours.
- Fees are applicable to the requesting party.
- Restrictions:
- Only specific records are generally released to individuals.
- Release to parties other than the patient or authorized individuals requires a court order.
