DIRECT ADMISSION
All Direct Admission-Direct to Room patients shall pass by the Admitting Department for admission data or, if beyond admitting office hours, be facilitated by the ER-assigned nurse for admission and shall undergo triaging in the ER, including RAT for symptomatic patients.
At the ER, Patients are received at the triage and referred to the Resident on Duty (ROD). The ROD assesses the patient and calls for the attending physician. Doctor’s orders are carried out before the patient is sent to the ward.
Upon admission, the patient will be asked about the room preference. The patient will give his/her information by providing valid IDs. Hospital rules, Patient’s Rights & Responsibilities, PhilHealth forms and requirements, and other documents will be discussed, and the representative will sign the Admission Checklist.
The process for admission follows the “One Call’ process that collects the essential patient information, including:
- Patient’s name, age, gender
- Contact information
- Reason for admission
- Medical history and current condition
- Referring provider’s contact information
- PHIC or Insurance Information
- PAG-IBIG Membership
- Government Issued ID
- Guarantee letter from assistance provider.
- Letter of Authority (LOA) from Health Maintenance Organization (HMO)
For Patients with Health Maintenance Organizations (HMO)
Patients are asked for a Request Letter of Authorization (LOA) or Letter of Guarantee (LOG) from the patient’s insurance provider or employer.
HMO cardholders swipe their cards to verify coverage and inform patients that hospital charges are still pending approval by their HMO.
